The annual transition conference hosted by the Alder Hey Transition team is increasingly recognised as an important event in the National Transition calendar. The highly successful conferences are regularly attended by over 100 delegates from across the UK with representation from professionals from health, education, social care and commissioning. We are particularly keen to engage with professionals from adult services and primary care whose active participation is essential to ensure the best possible transition experience for young people with long term conditions.
